Early U.S. CD release with discs in separate standard single jewel cases and same 12-page booklet (with lyrics, credits and artwork) in each (except one has "Double Disc Set Disc 1" and the other has "Double Disc Set Disc 2" on the front cover).  
Back-tray inserts display appropriate titles for individual CDs.
